Existing patients can request records by completing the Records Request Form. 

Request records

Please complete the Authorization Request for Release of Information form listed below (Required if you wish to obtain a copy of your dental record) and submit it to Dentistry Records. Call 614-292-5757 with questions. Please note: Patients utilizing MyChart can request records through their MyChart account (from June 17, 2025 onward). For any records prior to June 17, 2025, please use the Authorization Request for Release of Information listed below. To get started, sign into MyChart and navigate to the Document Center within the My Record activity section .
